You can report the loss or theft of your driver's license in two ways:

• Free online reporting to the RDW. For this, you will need DigiD.
   
• By appointment at the front desk. You will then fill out a missing persons declaration and pay a €24.50 missing persons fee.

Application after stolen/lost driver's license 

A replacement driver's license cannot be applied for online. 

• Apply for a replacement driver's license: by appointment at the front desk.
  Make an appointment
   
• Block driver's license (prior to your appointment)
  Block your missing driver's license online at the RDW (free of charge) or fill out a statement of loss (€24.50) during your appointment.
   
• Bring with you to the appointment:
  - color passport photo: not older than six months + meets the passport photo requirements
  - valid identification document
  - cash/pin card: you pay at the time of application
  - under 18 years of age: a companion pass (to be requested via 2todrive)
   
• Pick up driver's license: 5 business days later,
  Monday-Friday 10:00-12:30 am, Wednesday 2:30-7:00 pm.
  Do this within 3 months, otherwise you will need to reapply.
   

Rush request 

An urgent application is not possible online. For this you can go to the counter of the town hall. 

To do so, make an appointment by phone at 040-2631631. 

Driving licenses applied for urgently before 2:30 p.m. are usually ready the next working day (officially this is 48 hours). You pay an additional €39.65 for this. 

Health Statement

Have you lost a driver's license that required a Health Certificate when you applied? If so, keep in mind that your replacement license will have the same validity date as your lost license. Therefore, your replacement license will expire on the same date as the license you lost. 

Longer validity period

Would you like your replacement driver's license to be valid longer than the one you lost? Then you can apply for a Health Declaration. CBR will then reassess whether and for what period of time you may participate in traffic.
