Requesting a health certificate (CBR)
Note: Apply for the Health Declaration only through the CBR. If you end up on another website that does not require you to log in with DigiD then this website is not reliable!
When needed
You need a Health Statement when applying/renewing your driver's license:
- If you are going to take driving tests
- if your driver's license expires on or after your 75th birthday
- for medical reasons
- for a bus or truck driving license
(category C1, C, C1 E, CE, D1, D, D1 E or DE)
Your fitness to drive assessed
With a Health Declaration, you request an assessment from the Central Bureau of Driving Licenses. CBR then examines your physical and mental health. This often involves a medical examination.
If the CBR finds you fit to drive then you will receive a Certificate of Fitness. This means you are allowed to participate in traffic.
Validity
Period of validity of driving ability
If you receive notification from the CBR that you are medically fit to participate in traffic (Certificate of Fitness) this is valid for 1 year. This means that you have 1 year to renew your driver's license with us.
Driving license validity period
The period of validity that appears on your driver's license depends on the CBR's assessment. Some people are declared fit to drive for 5 years, others for 1 year. After this period, your license expires and you must submit a new Health Declaration to renew your license.
